Pathway Homes' programs across the continuum of care
Program Descriptions
Pathway Homes has a variety of programs which span the continuum of care, meeting individual support needs from highly supported to highly independent. All of Pathways' programs are licensed by the Commonwealth of Virginia and comply with the American Disabilities Act. The programs include:
- Stevenson Place Assisted Living Facility
Stevenson Place, which Pathway Homes manages under a contract with the Fairfax-Falls Church Community Services Board, is home to 36 men and women. This state-of-the-art facility provides 24-hour supportive mental health care as well as comprehensive services for medical, nursing and personal care needs.
- Discharge Assistance and Diversion Group Home
The Discharge Assistance and Diversion (DAD) program serves four men and four women in a community-based home with intensive 24-hour staff support, plus on-site nursing care.
- Group Homes
Pathway Homes operates two community group homes, one for eight women and one for eight men. They provide highly structures 24-hours-a-day, seven-days-a-week supervision and services.
- Semi-Independent and McKinney Homes
Pathways owns and operates six semi-independent houses serving 18 individuals. Staff are present at scheduled times, generally weekday afternoons and evening, and occasionally on weekends. Residents in this program possess independent living skills and receive less structure and supervision than those in the group homes
Another 56 semi-independent residents in fifteen townhouses are served through the McKinney program. Before becoming McKinney program participants, these residents had a history of homelessness. This program is funded by the Department of Housing and Urban Development (HUD).
- Supported Living
The Supported Living Program serves adults in their own Arlington or Fairfax homes. Counselors provide intensive case management services on a flexible and individualized basis.
- SHOP/Shelter Plus Care
Pathways most innovative program, serving more than 100 people, is the Supported Housing Options Program (SHOP). This program, a partnership with the Fairfax-Falls Church Community Services Board, employs consumers of mental health services in a variety of roles including planning and development and as mental health assistant counselors providing services to other consumers.
The goal of this program is to assist individuals in obtaining their own, independently leased permanent community housing and provide the support needed to maintain it. Such services are highly flexible and personalized.
The homes in which Pathways' programs operate are integrated into communities throughout Fairfax County and the Cities of Fairfax and Falls Church. Whether the homes are leased or owned, townhouses or stand-alone residences, Pathways takes great pride in presenting a positive, friendly image to neighbors. Staff and residents take part in neighborhood life and advocate for safe, welcoming communities. Above is the home of Pathways' Discharge Assistance and Diversion program.
Becoming a Resident
There are several criteria in order to qualify to enter a Pathways Homes program. They include:
- Being at least 18 years of age.
- Being a resident of Arlington County or Fairfax County, or the Cities of Fairfax or Falls Church, depending on the program.
- Having a history of serious and persistent mental illness.
There are two groups of people who are not accepted into Pathways' programs:
- Persons exhibiting behavior which would be a danger to themselves, the community or other residents.
- Persons actively abusing alcohol or other drugs.
